HCP LAN Launches Landmark APM Roadmap Tool

WASHINGTON, April 4, 2019 /PRNewswire/ — Yesterday, the Health Care Payment Learning & Action Network (LAN) launches its landmark tool, the Roadmap for Driving High Performance in Alternative Payment Models or the Roadmap. A customizable, interactive tool designed to help health care stakeholders navigate the challenges and opportunities inherent in Alternative Payment Model (APM) implementation. The Roadmap’s practical strategies and decision-making insights can accelerate payers’ efforts to design, implement, and expand or scale successful APMs.

The Roadmap is a capstone to the extensive portfolio of resources the LAN has built over the last four years, from the APM Framework to recommendations on designing clinical episode and population-based payment models, to data sharing, and primary care. This practical tool proudly reflects the ongoing efforts of the LAN, ever since its inception in 2015, to accelerate APM adoption. 

The Roadmap’s key insights emerged from direct conversations with both payers and providers who have successfully adopted and implemented APMs. Their experiences inform this specialized tool and reveal how the work being done by both payers and providers is successfully transforming the health care system from fee-for-service to value-based payments. An interactive tool, guidance repository, and resource center all in one, the Roadmap reflects the most current strategies and actions for driving down health care costs, while improving quality of care for patients. Its organization into three domains—APM Design, Payer/Provider Collaboration, and Patient Centered Care—enables a user to navigate directly to content that meets their personal needs.
