Signify Health Expands its Evaluative Services Technology Beyond the Home to Physicians’ Offices

DALLAS, April 16, 2019 /PRNewswire/ — Signify Health, the leading provider of technology-enabled healthcare services, has announced today a new evaluative services solution, Signify Office™, that will be used in physicians’ offices and will benefit health plans, health systems, and physicians alike.

Building upon a history of success with the in-home health risk evaluation, Signify Health is partnering with health plans to extend state-of-the-art technology and services into the primary care environment, equipping physicians and their staff to better document a complete and accurate clinical picture of their patients’ health year over year.

Signify Office, a web-based application, enables physicians to effectively and efficiently capture member health risk and quality data in a clinical office setting to optimize health plan prospective evaluation programs. As a comprehensive, protocol-driven tool, Signify Office empowers physicians to document a holistic picture of their patients’ health, from both a quality and diagnostic perspective.

During the pilot program in 2018, a select group of Signify Health clients implemented Signify Office for their physician office prospective program, with results as high as 65% patient panel completion. By utilizing the application, these health plans have combined their in-office program with existing in-home programs onto a single platform that drives consistency in data capture for CMS submission and supports value-based risk contracts for primary care physicians.

Each evaluation is configured based on the member demographics and health history to display all items that needs to be completed in a visit, including HEDIS and quality measures, which assist in the coordination of the best care possible for the individual. In addition, Signify Office provides real-time data syncing to accelerate the clinical documentation improvement and coding process, assuring accurate coding and data submission.
