Virginia Health Information Selects Collective Medical to Further Support Statewide Collaboration

The continued partnership ensures all hospitals participating in the EDCC program have a head start in meeting the CMS ADT notifications Conditions of Participation

RICHMOND, Va.–(BUSINESS WIRE)–Virginia Health Information (VHI) today announced its most recent service offering in partnership with Collective Medical to support broader ADT-based care coordination for hospitals, clinics, post-acute facilities and health plans across the commonwealth.

Through the continued partnership, hospitals participating in Virginia’s Emergency Department Care Coordination (EDCC) Program will be equipped with the tools needed to aid in compliance with the admission, discharge and transfer (ADT) notifications Conditions of Participation (CoP) included as part of the Centers for Medicare and Medicaid Services (CMS) Interoperability and Patient Access Rule. The new CoP, which takes effect May 1, 2021, requires hospitals to send electronic notifications to a new patient’s healthcare provider upon the patient’s admission, discharge or transfer. The notification may be sent via an intermediary, such as VHI.

“Virginia’s EDCC program has been a shining example of an HIE-led solution to improve the healthcare system,” says Michael T. Lundberg, VHI’s CEO. “Over the past few years, Collective has provided EDCC participants with the tools needed to drive clinical and financial outcomes for their most vulnerable patients. We’re excited to continue that partnership to provide hospitals across the commonwealth a connectivity solution to facilitate compliance with the CMS ADT notifications CoP.”

Collective now connects patient data across all 50 states. Physicians, nurses and other care team members benefit from patient insights sourced from every state in the nation. Care teams across all points of care are served by the Collective platform, including those based in hospitals, ACOs, emergency departments, physician practices, long-term and post-acute care facilities and behavioral health practices.

The new service builds upon the technology already in place to support the EDCC program, which all 106 of the commonwealth’s hospital emergency departments, as well as all MCOs, commercial and Medicare insurance carriers participate on the Collective network. According to an article by EHR Intelligence, after the first year of the program, fatal opioid overdoses in Virginia decreased from 1,230 in 2017 to 1,213 in 2018. This marks the first decrease after five years of increases in opioid deaths in the commonwealth.

“The new rules are a meaningful step toward achieving clinical interoperability and will certainly continue to support the transformation of care delivery in this country,” says Chris Klomp, CEO of Collective Medical. “We’re proud to continue to support VHI and the EDCC Program to demonstrate what can be achieved by enabling clinical collaboration.”
