Hospital and health systems chose CarePort platform over incumbent solutions throughout 2020

BOSTON–(BUSINESS WIRE)–CarePort, powered by WellSky®, a market leader in care transitions, today announced that more than 130 hospitals and health systems selected CarePort Care Management in 2020 over other discharge planning solutions, contributing to CarePort’s double-digit year-over-year client growth. With new health systems including Atrium Health and UMass Memorial Medical Center now using its platform, CarePort has significantly expanded its acute and post-acute footprint across the United States. The company’s increased scale benefits all providers on the CarePort network by enabling more opportunities for referral and care coordination with new providers.

In 2020, 41 of the more than 130 hospitals that CarePort added to its client roster chose CarePort instead of continuing contracts with incumbent care transitions solutions. According to clients, they chose the CarePort platform because of its key differentiators –– including its comprehensive post-acute database and referral network, post-acute analytics, patient choice tools, and closed loop referral capabilities –– all of which are integrated into the electronic health record (EHR).

“Post-acute network management is a critical initiative for our health system, and CarePort’s robust data and analytics have provided us with enhanced visibility into our network, strengthening our relationships with providers to improve discharge planning processes, and reduce post-acute length of stay and rehospitalizations,” said Kim Barry, RN, MS, CMAC, FACHE, Associate Chief Nursing Officer, Patient Care Services at UMass Memorial Medical Center. “CarePort continues to expand its integration with our EHR, which also influenced our decision as we were looking for a comprehensive, innovative solution that adapts with our organization as we grow and evolve.”

Clients also cited CarePort’s ability to efficiently track patients in acute and post-acute care settings –– particularly those receiving home health care –– as a critical reason for selecting the platform. As patients increasingly opt for in-home care, the ability to monitor patient progress from start to finish of home-based services is essential to fostering positive outcomes.

“In evaluating potential partners to optimize our health system’s transitions of care, CarePort’s end-to-end solution differentiated itself from other offerings on the market,” said Andy Crowder, Senior Vice President and Chief Information and Analytics Officer, Atrium Health. “The platform’s real-time data and alerts, as well as its integration with the EHR, will streamline our care management workflows and enhance our discharge planning process, giving us visibility into the activation of the post-acute service –– particularly home health care.”

In addition to expanding its client base, CarePort sent 20 million referrals to post-acute providers, representing an 18.4% year-over-year increase in referrals from 2019 to 2020. CarePort captures more than 30% of transitions to post-acute care and offers the largest end-to-end care coordination network that exists today, connecting more than 1,000 hospitals and 110,000 post-acute providers – including skilled nursing facilities (SNFs), home health agencies (HHAs), long-term acute care hospitals (LTACHs), inpatient rehabilitation facilities (IRFs), and hospices.

“As evidenced by our client growth over the past year, CarePort has proven to be an essential solution that enables healthcare providers to meet their care transition challenges,” said Dr. Lissy Hu, CEO and founder of CarePort. “As we look at the year ahead, we’re excited to continue to innovate and work toward ensuring high-quality and efficient care transitions for all patients by making meaningful investments in our platform, and collaborating further with new parent company WellSky.”
