The abstract, submitted by a California health plan, is entitled “Utilizing Mobile Technology To Improve Access To Timely Follow-up Care After Hospitalization For Mental Illness.” With the goal of increasing follow-up care within 7 days of hospital discharge, the California health plan implemented a novel way to connect with individuals recently discharged from psychiatric inpatient facilities.
