Expansion includes the addition of nearly 2,500 post-acute clients and 13,000+ home- and community-based providers to the network
BOSTON–(BUSINESS WIRE)–CarePort, powered by WellSky, a market leader in care transitions, today announced that nearly 2,500 post-acute providers and 13,000 home- and community-based services organizations joined the CarePort network in 2021, signifying record growth within the platform’s post-acute, home, and community client base. CarePort additionally launched CarePort Community, a new referral and care coordination platform for post-acute, home-, and community-based providers that added more than 220 clients within the past 12 months.
In today’s shifting healthcare environment, and especially as a result of the COVID-19 pandemic, post-acute and home- and community-based services organizations face an influx of medically complex patients amidst an industrywide staffing shortage. With more high-acuity patients in their care, post-acute providers must ensure that these patients are set up with the right services when returning to their homes.
CarePort Community was introduced in 2021 to address these care coordination needs. With more than 220 post-acute facilities on the platform, providers are using CarePort Community to electronically refer patients for home- and community-based services and ensure a successful recovery after a nursing home, long-term acute care, or acute/inpatient rehabilitative facility stay.

With CarePort Community, post-acute providers have access to 130,000 providers across CarePort’s existing national network, which continues to expand and adapt as patient and provider needs evolve. CarePort added more than 13,000 home- and community-based providers to the platform in 2021 – including physical therapists, occupational therapists, meal providers, transportation services, home medical equipment (HME), and durable medical equipment (DME), among others – to achieve better, more coordinated medical and non-medical patient care across the continuum. For example, through its recent partnership with Vertava Health, CarePort expands patient and provider access to high-quality treatment for substance abuse disorders – helping ensure all aspects of a patient’s health are part of the care journey.
Providers are also using CarePort Referral Management to simplify and streamline care transition processes. CarePort Referral Management results in a 67% increase in referrals received for skilled nursing facilities, and a 17% increase in referrals received for home health agencies. The average referral response time of a provider using CarePort Referral Management is 15 minutes, and the solution enabled nearly 25 million referrals in 2021.
