BOSTON–(BUSINESS WIRE)–CarePort, powered by WellSky, a market leader in care transitions, today announced that through partnerships, the CarePort platform has significantly expanded its data connections across the U.S., providing expanded visibility into a growing number of providers and increased care coordination capabilities across the continuum. Now connected to 17 heath information exchanges (HIEs) and data sources in the country and growing, CarePort simplifies the sharing of important patient data. CarePort’s clients can access admission, discharge, and transfer (ADT) event notifications and Continuity of Care Documents (CCDs) within the CarePort platform, allowing for:

• Additional clinical data including labs, medications, results, and procedure information
