Montage Health introduces new, innovative at-home patient monitoring service

The remote monitoring service advances quality and access of care for at-home patients with chronic conditions.

MONTEREY, Calif.–(BUSINESS WIRE)–Patients will soon benefit from improved quality and access of care with Montage Health Connected Care, an at-home patient monitoring service powered by Cadence that provides preventative care to patients with chronic conditions. The service will be provided at Montage Medical Group facilities for Medicare patients, lowering costs across the care continuum and helping providers deliver more efficient care.


Montage Health’s at-home patient monitoring program will initially support patients with hypertension, congestive heart failure, and type 2 diabetes, and will later expand to include other conditions.
