Group to Guide Platform for Streamlining Outpatient Care Coordination
FAYETTEVILLE, NY / ACCESSWIRE / April 2, 2024 / Ursamin, an emerging healthcare technology company dedicated to advancing the field of care coordination for patients with multiple chronic conditions (MCC), announced the formation of its Advisory Board.
The Advisory Board includes leaders in primary care, population health, care management and value-based care. The advisors will collaborate with the Ursamin leadership team to develop a platform tailored to the needs of family medicine and internal medicine practices, driving better care collaboration within primary care practices to manage patient populations, reduce costs and improve outcomes.
People living with MCC (PLWMCC) account for a disproportionate share of healthcare utilization and costs, 64% of all clinician visits, 70% of all inpatient stays, 83% of all prescriptions, 71% of all healthcare spending, and 93% of Medicare spending.[1]
The Agency for Healthcare Research and Quality (AHRQ) envisions a sustainable healthcare system that delivers high-value, coordinated, integrated patient-centered care, based in primary care, that will optimize individual and population health by preventing and effectively managing multiple chronic conditions.[2]
The advisors want to help change the trajectory. “Patients with multiple chronic conditions have a huge impact across the healthcare system, and care coordination has become an imperative,” said Shannon Aylesworth, CEO and Founder of Ursamin. “Our new advisors understand these forces firsthand and will provide invaluable perspectives to help us strengthen our platform and focus on areas that drive the most value for family medicine and internal medicine practices. We’re thrilled to have their guidance and support.”
Ursamin is committed to improving the lives of patients living with multiple chronic conditions by empowering primary care physicians and specialists with a care coordination platform that streamlines operational workflows – benefiting providers, caregivers and patients. The Advisory Board will play a pivotal role in ensuring Ursamin is at the forefront – delivering solutions that enable seamless collaboration and with a goal of achieving better patient outcomes.
