Group to Guide Platform for Streamlining Outpatient Care Coordination

The Advisory Board includes leaders in primary care, population health, care management and value-based care. The advisors will collaborate with the Ursamin leadership team to develop a platform tailored to the needs of family medicine and internal medicine practices, driving better care collaboration within primary care practices to manage patient populations, reduce costs and improve outcomes.

People living with MCC (PLWMCC) account for a disproportionate share of healthcare utilization and costs, 64% of all clinician visits, 70% of all inpatient stays, 83% of all prescriptions, 71% of all healthcare spending, and 93% of Medicare spending.[1]
