Statewide Initiative Launches Survey on Diabetes and Community Health Concerns to Advance Health Equity

LAS VEGAS, NV / ACCESSWIRE / September 4, 2024 / Comagine Health is excited to announce the launch of the Advancing Health Equity to Address Diabetes (AHEAD) Community Health Concerns Survey. This initiative aims to understand health concerns and perceptions of diabetes, focusing on its impact on the quality of life for Nevada residents. It is funded by the Centers for Disease Control and Prevention and supported by the Wellness Integrated Network of Nevada (WINN).

“We recognize that the number of people living with diabetes is on the rise, and navigating life with this chronic condition can be particularly challenging,” said Jerry Reeves, M.D., Comagine Health Medical Director. “Local residents who complete this survey will help us identify and address key issues, ultimately improving access to diabetes care for communities across Nevada.”

The survey, which is open September 1 through October 15, 2024, will help partnering organizations:

Community members who complete the survey will be entered to win $25 gift cards.

Comagine Health is a leader in community health work and collaborates with partners to improve health equity across the nation. Through partnerships across Nevada, Comagine Health and WINN are working to enhance access to the National Diabetes Prevention Program and Diabetes Self-Management Education and Support services across the state. The survey will be an important tool to help expand the reach of those programs.
