Biofourmis Selected by AdventHealth to Support “Whole-Person Care” With New Acute Care-at Home Program
NASHVILLE, Tenn.–(BUSINESS WIRE)–Biofourmis, a leading global technology-enabled care delivery company, and AdventHealth, have agreed to collaborate on a continuum-wide remote patient management (RPM) program in Central Florida to provide safe, whole-person care and a superior patient experience within their homes.
As part of the agreement, Biofourmis will help AdventHealth expand whole-person care into the home with the AdventHealth Hospital at Home program where the Biofourmis platform and remote clinical care team will support inpatient-level care for patients at home. Biofourmis’ enterprise platform enables complex, personalized, holistic care at home through market-leading capabilities such as continuous monitoring and FDA-cleared AI algorithms, which will support AdventHealth in delivering and scaling its care at home strategy while ensuring an outstanding care experience. The program will launch at AdventHealth Winter Park.
Unique to Central Florida, AdventHealth is looking to standardize the patient experience by also offering post-acute RPM to keep patients engaged in their health with a seamless patient experience across hospital at home and after discharge remote patient management for continuity of care.

To further improve the experience, Biofourmis is seamlessly integrating the health system’s Epic electronic health record (EHR) system to ensure critical patient data is integrated for continuity of care to help clinicians efficiently manage acute care in the patient’s home.
In addition to the continuous monitoring, the Biofourmis platform includes integrated personal emergency response system, as well as arrhythmia and fall detection, to protect patients and to build confidence among inpatient clinicians that patients will be safe at home during their care. AdventHealth will also have the ability to schedule, coordinate and track in-home ancillary services through The Biofourmis Platform to enable greater efficiency and management of whole-person care at home. The collaboration is designed to ease and accelerate AdventHealth’s ability to scale its care-at-home to serve more patients.
