This deployment milestone will facilitate expanded data integration across its ACO participating providers, accelerate required quality outcomes reporting, and aggregate data management and analytics across the ACO partner’s value-based care initiatives.

With 10Bridge, the ACO partner can access consented patient data, generate dashboards, and track various care gap metrics, including in- and out-of-network utilization, post-acute care, clinical episode data, and risk-adjusted factor (RAF) scores. This data integration supports the ACO partner’s mission to improve patient outcomes and close care gaps while maximizing shared savings opportunities.
