PointClickCare Now Supports Over 32.7 Million Californians with Expanded Care Collaboration Network

Industry-leading vendor grows its investment in California, offering the most expansive, full-continuum care collaboration network statewide

TORONTO–(BUSINESS WIRE)–PointClickCare, a leading healthcare technology platform enabling meaningful care collaboration and real‐time patient insights, is announcing the details of its continued investment and expansion across the state of California. Through the integration and standardization of California’s largest collection of encounter and clinical data, PointClickCare is trailblazing the path to more streamlined care for all patients statewide. With a Care Collaboration Network that covers 182 hospitals, 229 ambulatory clinics, 1,054 skilled nursing facilities (SNFs), and 17 health plans, the company’s network now supports over 32.7 million Californians, and 55% acute and 78% SNF providers across the state that currently share data through its platform.

“This network uniquely positions us to continue to support and expand use cases that drive the future of California’s Medicaid transformation and care coordination priorities like CalAIM’s Enhanced Care Management (ECM),” said Brian Drozdowicz, Senior Vice President and General Manager of Acute and Payer Markets at PointClickCare. “As our breadth and depth across the state continues to grow, stronger data integration for regulatory compliance is top of mind for us to ensure high needs patients receive the quality care they deserve, while improving overall efficiency and cost of care in California.”

Since 2018, PointClickCare has been pioneering interoperability to provide better care coordination and deliver improved outcomes for Californians, delivering access to the right real-time insights at the right time. As the company’s investment in California grows, so does the functionality of its solutions in supporting patients in the emergency department (ED) and beyond. Enhancements include:

  • Streamlined SNF Transfers: Improve communication between SNFs and emergency departments, improving transitions and patient outcomes by delivering critical clinical insights directly to the ED track board upon a patient’s arrival from a skilled nursing facility.
  • Enhance Care Delivery for High Priority Populations: Swiftly identify high-risk, complex patients such as those with substance use disorder, mental health issues, readmissions, or social determinants of health (SDoH), improving care coordination and enabling prompt and informed care decisions.
  • Integrated into Clinical Workflow: Seamlessly integrate PointClickCare network features, alerts, and actionable information directly into the EHR workflow through SMART on FHIR, reducing double documentation and ensuring clinicians access actionable insights and pertinent information promptly for improved clinical outcomes.

“PointClickCare has helped us evolve and improve how we support our emergency department (ED) patients,” said Dr. Ronn Berrol, Medical Director, Emergency Services at Sutter Health Alta Bates. “We’ve been able to reduce high utilizer visit frequencies and they are no longer taking up beds for six or more hours. If a high-needs patient presents to the ED, we can quickly and effectively triage, so that individuals are in and out in around 40 minutes. We had one patient with more than 200 ED visits last year, and now that we have better care insights and can immediately recommend other outpatient resources, that individual has had zero ED visits this year.”

Access to healthcare data alone is not enough to truly impact the lives of high needs patients, which is why PointClickCare’s solution extends beyond simple data exchange. Health Plans and ECM providers across the state can harness the power of care collaboration across the entire care network – and the results are tangible.

“Once we implemented PointClickCare, our teams and our case managers were able to receive real time notifications and ADTs, and we are able to proactively perform outreach to those members and offer services and care,” said Gema Salas, Health Information Ecosystem Program Manager at LA Care.

In the coming months, PointClickCare will be attending industry events across the state to continue helping California providers collaborate and improve outcomes, including:
