The Serious Illness Performance Collaborative: Leading Palliative Care Organizations to Use Advanced Data Science to Succeed in Value-Based Care

The goal of the Collaborative is to better equip palliative care providers with the tools, skills, and experience required to succeed in value-based contracts such as the Center for Medicare and Medicaid Innovations (CMMI) Primary Care First – Seriously Ill Population (PCF-SIP), Direct Contracting – High Needs Population, and Value Based Insurance Design (VBID) models. Its initial focus will be on success within the PCF-SIP program, in which one-third of reimbursement will be contingent on performance against a set of process and outcome measures. 

To maximize their success, participants in the Collaborative will benefit from machine learning models designed specifically to support the contract’s successful transitions and cost savings requirements. The learning collaborative will include guidance from experts and participants will share data and best practices with each other.  

It’s a timely goal, according to Caitlin Brennan, Ph.D., a Nurse Practitioner and palliative care clinician who is spearheading the initiative for Cyft. “For decades, the impact of palliative care has been limited by the lack of sustainable revenue models. Right now, we have the opportunity to change that.”

Dr. Brennan will be joined by Peter Benjamin, Martha L. Twaddle, M.D., Gary Bacher, and a host of other experts in guiding the Collaborative’s content. “If enough organizations demonstrate high-quality care that prevents unnecessary medical spend, commercial payers and CMS will follow with more value-based opportunities. We believe the best way to make that happen is to support one another in making the most of early value-based care opportunities,” said Mr. Benjamin.
