The Serious Illness Performance Collaborative: Leading Palliative Care Organizations to Use Advanced Data Science to Succeed in Value-Based Care

CAMBRIDGE, Mass., March 10, 2020 /PRNewswire/ — Cyft Inc., a health care analytics company dedicated to helping organizations succeed in value-based care, has announced the launch of the Serious Illness Performance Collaborative. Participating organizations will benefit from advanced data science as well as the support of a learning community.

Several leading palliative care organizations have already joined the Collaborative, including:

  • Ohio’s Hospice and their affiliates
  • Bluegrass Care Navigators (KY)
  • Care Synergy and their affiliates (CO)
  • Hospice of Northwest Ohio
  • Empath Health (FL)
  • Care Dimensions (MA)
  • HopeHealth (RI and MA)
  • Snowline (CA)
  • Capital Caring Health (DC, MD, VA)
  • Trustbridge (FL)
  • Midland Care (KS)
  • Delaware Hospice
  • Cornerstone Hospice & Palliative Care (FL)
  • Community Hospice & Palliative Care (FL)
  • Catholic Hospice (FL)
  • Hope Healthcare (FL)

The goal of the Collaborative is to better equip palliative care providers with the tools, skills, and experience required to succeed in value-based contracts such as the Center for Medicare and Medicaid Innovations (CMMI) Primary Care First – Seriously Ill Population (PCF-SIP), Direct Contracting – High Needs Population, and Value Based Insurance Design (VBID) models. Its initial focus will be on success within the PCF-SIP program, in which one-third of reimbursement will be contingent on performance against a set of process and outcome measures.

To maximize their success, participants in the Collaborative will benefit from machine learning models designed specifically to support the contract’s successful transitions and cost savings requirements. The learning collaborative will include guidance from experts and participants will share data and best practices with each other.

It’s a timely goal, according to Caitlin Brennan, Ph.D., a Nurse Practitioner and palliative care clinician who is spearheading the initiative for Cyft. “For decades, the impact of palliative care has been limited by the lack of sustainable revenue models. Right now, we have the opportunity to change that.”

Dr. Brennan will be joined by Peter Benjamin, Martha L. Twaddle, M.D., Gary Bacher, and a host of other experts in guiding the Collaborative’s content. “If enough organizations demonstrate high-quality care that prevents unnecessary medical spend, commercial payers and CMS will follow with more value-based opportunities. We believe the best way to make that happen is to support one another in making the most of early value-based care opportunities,” said Mr. Benjamin.

“Advanced analytics such as machine learning will play an important role in helping our clinical teams prioritize limited resources and match individuals to the appropriate care pathways,” said Anthony Evans, President of Pure Healthcare & EVP of Ohio’s Hospice. “We look forward to participation in this collaborative which will allow us to further develop these next-generation capabilities.”

Liz Fowler, President & CEO of Bluegrass Care Navigators, noted “Technology is important, but it’s how you use it to support clinical decision-making that matters. For us, there’s a significant advantage in learning from industry experts and peers to identify best practices for putting these insights to work. That is what sets the Serious Illness Performance Collaborative apart.”

Organizations planning to participate in PCF-SIP, VBID, Direct Contracting, or custom value-based models with payers that are interested in learning more should contact Bob Tavares at btavares@cyft.com or visit www.cyft.com.
